What can we notarise during the restrictions in Greater Sydney? (July 2021 update)

We are currently closed for face-to-face appointments

Thanks for your continued interest in our services and our support. Unfortunately, due to the extended restrictions across Greater Sydney, our office is still closed to face-to-face appointments until further notice.

What does this mean?

We're still providing legal and notary public services, however, they're limited to services that don't require face-to-face appointments. These services include:

  • Attesting or certifying copies of documents. Generally, this still means that you must arrange for the original document to be delivered to our office*. We do NOT recommend posting important original documents like passports but if it's urgent and absolutely necessary, then you should arrange for your document to be couriered to our office. Once notarised, our office will arrange for a return courier to ensure that your original documents and notarised copies are returned to you safely and securely.
  • Verifying documents. If you have certain electronic documents that must be verified (such as Australian visa documents (ie, VEVO), Australian educational documents, Australian medical documents, Australian employment documents), these documents can be verified (before being notarised) with whoever issued the documents. Alternatively, if these documents are not electronic documents and there are only original documents, then the same applies as attesting and certifying. We require the original documents to be delivered to our office* before we can contact whoever issued the documents to verify them.

*If you personally deliver original documents to our office, you may be subject to a fine for breaching the current 'stay-at-home' orders. Any documents must be able to be slipped under our door as our office will NOT receive you or open the door for you when you arrive.

Accordingly, we can only recommend that you arrange for your document to be sent by courier (or if you are not concerned that the document could be lost, arrange for it to be posted) otherwise we suggest waiting until the restrictions have been lifted.

Importantly, during the current restrictions, we are not providing services that require a face-to-face appointment, mainly this refers to services that include witnessing. This means we cannot notarise documents such as:

  • Power of attorney, both general power of attorney and special power of attorney.
  • Declarations and affidavits (including the Indian Drivers Licence Verification)
  • Passport renewal forms such as Form V, Form I, or Form J
  • Australian visa renewal such as Form 1229
  • Life certificates
  • Court documents

For this service or for any of these documents that require witnessing or a face-to-face appointment, you must wait until the restrictions have been lifted before calling our office to make an appointment.

Update

Restrictions for Greater Sydney have now been extended to 30 July 2021.

Form V - Can you help me complete this form?

Lost your Indian passport or damaged your Indian passport?

If you've lost or damaged your Indian passport, you'll need to apply to VFS for a replacement. The way you do this is by submitting Form V (please make sure you visit the VFS website to obtain the latest version of this form as it could change without notice).

FORM V, also known as the AFFIDAVIT FOR A PASSPORT IN LIEU OF LOST/DAMAGED PASSPORT, requires you to complete the details contained in the affidavit and then declare and sign it in front of a notary public - that's us!

What information do you need for FORM V?

FORM V contains a number of questions that you must answer. If you don't properly answer the questions or provide the information necessary, your form could be rejected.

I S/o, D/o, W/o Shri... residing at ...

Solemnly affirm as follows:

1. State how and when the passport was lost/damaged and when FIR was lodged at which Police Station and how many passports were lost/damaged earlier?

2. State whether you travelled on the lost/damaged passport, if so state flight number and date and port of entry into India?

3. State whether you availed of any TR concessions/FTS allowance and if so details thereof?

4. State whether non-resident Indian and if resident abroad, the details of the residence as follows:

  • Name of the Country
  • Length of Residence from ...to...
  • Page Nos. of passport bearing departure and arrival stamps

5. State whether the passport had any objection by the PIA and if so the details thereof.

6. State whether you were deported at any time at the expense of the Government and if so was the expenditure incurred reimbursed to Government of India.

I further affirm that I will take utmost care of my passport if issued and the Government will be at liberty to take any legal action under the Passport Act, 1967, if the lapse is repeated.

FORM V - No Original Photograph Identification

What is Form V?

Form V is the Affidavit in Lieu of Lost or Damaged Passport. It is the form required by the VFS, on behalf of the Indian Consulate, that you must complete in order to apply for a new/replacement Indian passport to replace your passport that has been lost or damaged.

Form V identification requirements

Before we can assist you with the FORM V, you must be able to prove your identity. You can only do this by providing us with original photograph identification such as:
  • Indian Passport. If your Indian Passport is damaged, your photograph and your name/date of birth must clear and identifiable - you should contact us if you have any concerns whether your Indian Passport can be used to prove your identity.
  • Other passport. If you hold another passport, in addition to your Indian Passport, you may use that passport.
  • Australian Drivers Licence or Photocard. You must have obtained this from a driving authority in Australia.
  • Other Australian Government issued photograph identification. Please contact us and provide us with a copy of your proposed photograph identification for confirmation.
The kinds of identification that we do NOT accept include:
  • Photocopies or certified photocopies of a passport or any other original photograph. We will only accept original photograph identification.
  • Medicare card. The Medicare card is a form of Australian Government identification however it does not include a photograph.
  • Indian Drivers Licence or PAN card. We are unable to identify you in Australia by non-Australian identification documents.
  • Police Report. You may receive a Police Report if you have reported that your Indian Passport has been stolen, however we do not accept this as a form of identification.
If you do not have the necessary photograph identification document, you should contact VFS directly and advise them of your issue and ask for guidance as to your other options.

What do I need to bring to get FORM V notarised?

If you have lost or damaged your Indian Passport, or if it has been stolen, you will need to complete and submit FORM V to VFS to have a new passport issued to you.

As a leading provider of notary public services, many clients approach us for assistance because FORM V must be notarised - it cannot be signed by a justice of the peace.

Accordingly, to have FORM V notarised, you must attend our office and prove your identity in order to have your signature witnessed. This means, if your passport has been lost, stolen or damaged beyond recognition, you must have some other form of photograph identification that is normally acceptable in Australia (for example, an Australian drivers licence or photocard issued by the RTA/RMS). Other forms of non-Australian photograph identification will not be accepted.

Unfortunately, we have had to turn away many people without being able to assist because they did not have the required identification documents to prove their identity. If you do not have your passport or any the form of Australian photograph identification, you should contact VFS for clarification on your options.

Summary of Requirements


  1. Appointment. Make an appointment to have your documents notarised. You should also request a quote for the notary public service. Please call 02 9687 8885 between 9am-5pm Monday to Friday, excluding public holidays.
  2. Form V. Complete the information required on the form before your appointment. Do not sign it until asked. We cannot assist you with completing the information on the form.
  3. Photograph ID. Bring your passport, or if your passport is lost, stolen or damaged beyond recognition, you must bring another form of Australian identification (ie, Australian drivers licence or photocard).

Notarising Form V

How annoying and frustrating is it to lose your passport or have your passport damaged so that it's no longer valid? If you're an Indian national in Australia, then the process to have your passport replaced is fairly straight forward if you know what to do, and if not, at least you know where to look to find the relevant information.

If you're an Indian national, then applying for a replacement passport when you've lost or damaged your current passport is done through VFS Global - like many other consular services. A quick review of their website will explain the process and the checklist will help you compile the documents necessary to have a 'hopefully' easy/simple application experience. As you work through the process and the checklist, you'll find that a variety of documents and forms need to be attested by a justice of the peace, however once you get to Form V, you'll find that this form must be signed in front of a notary public and notarised. That's where we come in.
